How to Become A Fresh Mushroom Farm Customer

We Look forward to doing business with your restaurant, club, or hotel! It's easy to become one of our customers. Please complete the following steps:

1. Complete a credit application. Please request a credit application from us and fill in as much information as possible (the more information that's available to us, the more effeciently we can process it for you). And please do not forget to sign the credit application before returning it to us.

2. Complete the Missouri sales tax exemption certificate. The instructions are included with the form. Don't forget to sign this form also.

3. Return the completed forms to Fresh Mushroom Farm, Inc. (address located on our Credit Application) and within seven to ten days you will hear from us. If we need more information from you we will request it, and when your credit is approved we'll allocate a credit limit for you and can place an order with us on that same day!
